How to Enroll
If you have original Medicare and want to know if you qualify for GUIDE services, Balance Health Plan can help. Our care navigators will complete an assessment to see if you meet the criteria to enroll. If you do, we’ll guide you through enrollment and ongoing participation. You’re welcome to get a referral from your primary care provider or neurologist, but it’s not required.
