What is Guide
The GUIDE Program, formally known as the Guiding an Improved Dementia Experience (GUIDE) Model, is a new Medicare-funded initiative spearheaded by the Centers for Medicare & Medicaid Services (CMS). This program aims to improve dementia care for individuals living with the condition, while also providing support for unpaid family caregivers.
Finding Balance with the GUIDE program
Our program supports individuals living with dementia in staying safely at home for longer. GUIDE helps reduce avoidable hospitalizations and emergency room visits. We offer both emotional and practical support to ease the burden on caregivers.
